I have a book in PDF form that I have already edited with Adobe Acrobat to include bookmarks for each chapter. When I use the PDF on my Kindle, however, these bookmarks are not present. I can certainly spend time adding bookmarks again, but the problem is, now they're all "Page 8," "Page 189," etc, not "Chapter 1" or "Appendix A." How can I get Kindle to view these bookmarks I created, or alternatively how can I create a TOC in the original PDF that the Kindle will recognize?

Or is it even possible to accomplish this with a PDF file? Does the Kindle only support TOC's for .azw files?
