Did you try 52 Novels? They're extremely responsive, dilligent and even came in a few weeks ahead of the original timeframe they quoted me.

Depending how complicated your formatting needs to be, you might be satisfied with the free book conversion through Draft2Digital. I use them for everything I publish.
